ALMA-Clinics” (Alma), is a Primary healthcare (PHC) Nurse-led Medical Technology Clinic Network that leverages technology to make comprehensive Primary care accessible, affordable and offer effective treatment in order to reduce consequential complications related to non-communicable diseases in low-income market segment. Alma offers PHC services through a Digital PHC platform (ABEOBARE) that seeks to create empowerment of patient and nurses, shifting healthcare from curative to preventative , and validating a primary healthcare system that empowers all stakeholders with the “patient” at the centre and more a transparent system. It offers a solution that will respond to current healthcare challenges and create opportunities for the South African market, enabling millions of people to have access to quality affordable care. The solution leverages innovative portable diagnostics and an AI-driven digital platform technology to enhance services at primary healthcare through early diagnosis and treatment of diseases to reduce burden at tertiary care and prevent early mortality and disabilities associated with NCDs. Consequently, it will empower nurses to cater to more patients in an efficient manner and increase health literacy through the mobile app where patients will take ownership of their health. Professional, Ethical and Legal Conduct Collaborates/consults with peers and other health professionals regarding patient assessment, diagnose and management; Takes responsibility for own case load of (client) health care users and practice, including diagnosis and prescribing practice; Accepts personal and professional responsibility/ accountability for legal consequences of own diagnosis (assessment) and prescribing practices; Works within National Health Care Systems codes of conduct related to treating patients, including prescribing oral medicines; Practices own profession according to Code of Ethics/conduct and standards related to primary clinical care; Identifies ethical dilemmas and make a critical and responsible decision, taking into consideration all legal and ethical frameworks; Compiles and keeps accurate clear and complete records of the assessment, diagnosis and management of the patient. Clinical Assessment Conducts a comprehensive and holistic health assessment on clients of all age groups, with complex health problems (acute, chronic and emergencies); Reflect all relevant information and patient records within Alma E health App; Timeously identify high-risk situations and conditions in a patient and take appropriate action; Conducts a rapid appraisal and assessment of emergency conditions; Determines patient's history, both medical and medicinal; Interprets history and identifies health problems and makes differential diagnoses; Conducts skilfully, an advanced physical examination utilizing the four basic assessment techniques (inspection, palpation, percussion and auscultation) using relevant equipment; Synthesizes and interpret history and physical examination findings to make an appropriate diagnosis and or health problem; Performs and requests diagnostic investigations as justified by the patient's health status/condition, protocol and authorisation, liase with the doctor to interpret investigation results to confirm the final diagnoses. Clinical Diagnosis Interprets history and physical examination findings to identify a health problems and makes differential assessment; Orders laboratory tests and perform diagnostic procedures, relevant to findings in history and physical examination findings; Applies in depth knowledge of anatomy, physiology and patho-physiologyin assessing, and management of specific conditions; Assesses multiple pathologies to make differential diagnosis; Makes specific diagnosis (final diagnosis) Interpret results of relevant special investigations in common conditions in consultation with the supervising Clinical Associate and Doctor. Clinical Treatment Discusses with patient the assessment, treatment and possible complications, to obtain participation and increase compliance; Discusses patient responsibilities towards achieving expected short and long term treatment outcomes; Provides appropriate pre-referral/emergency treatment to stabilize patient; Communicates clearly to the patient her/his specific individual follow-up plan (including urgent/immediate return) for the continuation of care; Prescribes relevant medication as per applicable legislation and protocols, taking into account prescriber's responsibility and accountability (rational drug prescribing) including care plan; Apply in-depth knowledge of pharmacology when considering drug management of a specific patient; Prescribing according to competency level and authorization; Provide clarity on medication action, including but not limited to correct dosages, desired action, interactions, contra-indications and adverse effects; Adheres to all principles of prescribing that meets legal requirements (legibility, clarity, completeness); Refers appropriately clients/patients with complicated health problems to the next level of health care through guidance of Doctor; Evaluates progress and treatment response; Adjusts treatment plan as required by patient's condition; Checks patient's understanding and commitment or compliance to treatment regime; Identifies and manage potential complications of the presenting condition, as well as side/adverse effects of the drugs; Applies effective therapeutic communication skills throughout consultation process (assessment, diagnosing, prescribing and referral). Health Literacy Utilise effective listening in order to explain diagnosis with empathy. Share health information in appropriate cultural and language terms. Plans and implement life style adjustment/management according to the specific health needs of the client in consultation with the client Explain and/or describe the procedure in lay and medical terms. Explain indications, contra-indications, and possible complications for a specific procedure. Give appropriate amount and level of information. Use a suitable environment to communicate with the patient and/or family. Facilitate patient feedback and questions. Ensure confidentiality. Involve patient in appropriate health solutions. Facilitate mutual decision-making. Provide continual support and follow-up. Foster adoption of Alma E Helath and explain benefits to patient Quality Improvement Participates actively in the development of clinical protocols and guidelines related to area of practice Participates in setting of standards for clinical practice for nurse specialist Applies evidence-based practice to keep up-dated and skilled with the consultation and management of patients Engages in reflective clinical practice to improve care Audits patient records Engages in peer group mentoring and evaluation to improve quality Documents, shares and reports practices, including prescribing errors and near misses Searches and uses tools that improve prescribing practice Establishes professional links with other practitioners and prescribers Continuing Education Takes responsibility and complies with CPD requirements specific to Primary Care Nursing Practice as required by a professional/regulatory body Takes responsibility to update self on the latest legislation, policies, protocols, guidelines regarding the assessment, diagnosis and management of acute, chronic and emergency conditions, including prescribing Keeps updated and skilled with the latest/current diagnostic technology and other related technology Professional Leadership and Development Partakes in the development and change/review of health policies, related to primary care and prescribing Establishes working relationships with colleagues, including private practitioners to ensure that continuity of care is not compromised Negotiates the appropriate level of support required by her role as a primary care specialist and supplementary prescribe Directing and supervising the care of other nurses eg auxillary nurse staff nurse and assistant nurse Alma E health Digital Adoption Ensure that Alma E Health is used according to the SOP that define ALMA-Clinic Monitor the level of adoption of Alma E Health APP from patients Provide recommendations on strategies and action plans to improve the patients' adoption level Promote actions to foster patient's adoption of Alma E health APP Medical Equipment Ensuring that clinic medical devices and equipment is functioning optimally; Ensuring that clinic medical devices and equipment's inventory list updated, and reports are sent to Clinic Supervisor timeously; Using Medical devices correctly; Report any faults to Clinical Associate Ensure appropriate cartridges are available for medical devices Ensure that cartridges inventory management is updated at all times Conduct monthly staff training on how to use medical equipment and Alma E health APP Midwife, Antenatal and Ultra Sounds Valid Drivers License Ages between 32 - 38 years Bachelor Degree in Nursing Experience in Primary Care and Patients with Non-Communicable diseases Market Related Apply Now
