REMOTE HOME & COMMUNITY BASED SERVICES PROCESS CO-ORDINATOR AVAILABLE FOR ALL CANDIDATES PUBLISHED: 13 MAY 2024 My client based in New York is seeking a dynamic & compassionate individual to join their team as an Adult Behavioural Health HCBS Process Co-Ordinator. In this role, you will play a crucial part in facilitating the delivery of Home & Community Based Services for adult with behavioural health needs in compliance with New York State Regulations & guidelines. The position offers this remote work opportunity within the State of New York (USA). Requirements: Co-ordinate & oversee the administration of Home & Community Based Services for adults with behavioural health needs Conduct comprehensive assessments to determine eligibility for HCBS programmes and develop individualized service plans Collaborate with clients, their families, care givers and service providers to ensure co-ordinated & effective care delivery Monitor the implementation of service plans, including the provision of behavioural health treatments, support services & community integration services Provide ongoing support & guidance to clients and their support networks, addressing any barriers to service access / participation Maintain accurate and up-to-date documentation of client information, service plans and progress notes in compliance with regulatory requirements Serve as a liaison between clients, service providers & relevant agencies to facilitate communication & problem-solving Stay informed on changes in regulations, best practices & emerging trends in the field of behavioural health and HCBS Duties: Bachelor / Master's Degree in Social Work, Psychology, Counselling, or related field Previous experience working in behavioural health services / Centres, case management or co-ordinated programmes Knowledge of New York State regulations & guidelines governing Home & Community based services for adults with behavioural health needs Strong assessment, planning and communication skills Ability to work collaboratively with diverse populations and multi-disciplinary teams Excellent organizational skills with strong attention to detail Proficient in using electronic health records (HER) and other relevant software applications Flexibility to adapt to changing priorities & work independently in a remote environment Hourly rate in USD on offer. Working international New York times zones from your home office. You will need your own laptop / WIFI and internet connectivity.
