The Community Health Specialist promotes, maintains, and improves individual and community health by assisting individuals and communities to adopt healthy behaviors. Collect and analyze data to identify community needs for program planning, monitoring, and evaluating programs designed to encourage healthy lifestyles, policies and environments. May also serve as a resource to assist individuals, other professionals, or the community. JOB DUTIES AND RESPONSIBILITIES: Collaborates with community health staff and community organizations to determine community health needs and the availability of services, and to develop goals for meeting needs of assigned projects. Designs and conduct evaluations to assess the quality and performance of community health programs. Present health education and promotion programs to patients and community members. Identifies barriers, provide assistance and support to patients based on their individual needs relating to health education programs. Provides individual or group counseling and education to patients when necessary. Completes detailed documentation of all encounters and attempted encounters with patients. Assures complete data collection and recording in corresponding databases. Completes all reports in a timely manner. Maintains cooperative working relationships with hospital employees and community organizations interested in community health education programming. Coordinates and conduct outreach activities as needed. Acts as a liaison between client and other community health agencies/providers as needed. Monitors health education programs for effectiveness. Assures and monitors compliance with the ACoS CoC regarding the community outreach activities in Oncology. Presents an annual report to the Cancer Committee describing the results of the Oncology Community Outreach activities, ensuring all the CoC requirements are included. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patients served. Demonstrates an understanding of cultural and social barriers to the exchange of health-related information within the community. Maintains confidentiality of all materials handled within the Network/Entity as well as the proper release of information. Demonstrates/models the Network's Service Excellence Standards of Performance during interactions with all customers (internal and external). Complies with Network and departmental policies regarding attendance and dress code. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices. PHYSICAL AND SENSORY REQUIREMENTS: Sitting for up to 4 hours per day, up to 2 hours at a time. Stand for up to 6 hours per day, 3 hours at a time. Walk to 6 hours per day; 20 minutes at a time. Frequently use hands and fingers. Consistently lift, carry and push objects up to 25 lb. Frequently stoop and bend. Frequently reach above shoulder level. Must be able to perceive attributes of an object through touch. Must be able to hear as it relates to normal conversation, and high and low frequencies. Must be able to see as it relates to general, near, far, color, and peripheral vision. QUALIFICATIONS (MINIMUM): EDUCATION: Experience in community health/public health or a health/medical related field. Some college degree education is required. Education in Public Health is preferred. TRAINING AND EXPERIENCE: At least one-year experience in working with public health programs is required. Previous experience working with low-income and diverse populations preferred. Ability to engage and maintain constructive relationship within a diverse population. Bilingual in Spanish preferred.
