The Clinical Dietician (RD) provides patients, visitors, and co-workers with outstanding customer service. Dietitians will provide comprehensive, high quality nutritional care to patients in the hospital, clinics, and related entities (Hospice, clinics, dialysis etc.). This will include, but is not limited to: examinations, assessments and/or diet counseling-instructions for patients/families on assigned units. Each Registered Dietitian (RD) will make appropriate recommendations to physicians. Assessments will be documented electronically in the Medical Record or on the network paper nutrition assessment sheet. Other duties include research, teaching, and community service. JOB DUTIES AND RESPONSIBILITIES: Identified patients to be assessed through triggers of patients that are at potential Nutrition Risk or via physician consults or through disease pathways. Assess all patients on tube feedings or TPN with appropriate recommendations. Each assessment should include recommendation for substrate and macronutrient content. Design and implement an individual nutrition care plan for patients identified through the screening process to be at nutrition risk. Develop a specialized plan of care for each patient using a physical assessment, medical history, Lab data, drug profile, and diet history. Routine monitoring of TPN and tube fed patients weight, total fluid intake, fluid output, nutrition related lab values and tolerance of feeding modality. Diet instructions/counseling to patients according to established procedures and/or as ordered (in and outpatients). Family consultations scheduled when patient is unable to receive or retain information. Documentation of all assessments, instructions, counseling and "follow-ups" in patients' medical record (via computer). Completes routine follow-up on Level 3, 2 or 1 patients and assesses response to the nutrition care plan. Liaison between Nursing, Medical Staff and other support services i.e. speech, OT, case management. Educate physicians, nurses, and other professionals regarding medical nutrition therapy. Attend and advise at appropriate meetings according to assigned units. Ex: ICU, Trauma, Diabetes, Rehab, Wellness, Patient Education, Oncology, discharge Management meetings as requested. Participate in (or lead) departmental/hospital Quality Improvement activities. Attendance at Nutrition Committee (if needed). Participate in planning in-Service meetings for Medical and Nursing Staff, Dietitians, Technicians, and Clinical Aides. Revise and develop materials for Patient Education in area of expertise. (as requested) Attend or perform continuing education in order to maintain registration, PA license and competency. Involvement in department activities, Community programs, and activities. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development, and communication appropriate to the age of the patient treated. (Ongoing peer and year chart audits) EDUCATION: B.S. Degree certified as a Registered Dietitian. Active registration and PA state license maintained through state nursing board and the CDR. MS in Nutrition or health/counseling; related field desirable. TRAINING AND EXPERIENCE: Minimum One to Two years' experience in clinical nutrition.
